Cancellation Policy

Transform Dental – Cancellation Policy and T&C’s

At Transform Dental, we value your time and strive to provide the highest standard of

care. To ensure we can accommodate all patients effectively, we kindly ask that you

adhere to our cancellation policy.

Short Appointments

Appointments of shorter duration require a minimum of 48 hours’ notice for

cancellation or rescheduling.

Longer Appointments

Appointments of longer duration require a minimum of 7 days’ notice for cancellation

or rescheduling.

Late Cancellations & Missed Appointments

Failure to provide the required notice will result in a charge of 25% of the scheduled

treatment cost.

This policy allows us to offer appointment times to other patients in need of care.

Where a Patient chooses to fund treatment through a third-party finance provider, they

retain any statutory rights to cancel the finance agreement in accordance with the

provider’s terms and conditions.

For the avoidance of doubt, cancellation of a finance agreement does not cancel,

amend, or otherwise affect the Patient’s agreement with Transform Dental for the

provision of treatment. The Patient remains fully responsible for all fees and charges

relating to their Dental Treatment Plan, including any applicable cancellation or missed

appointment fees.

In the event that a finance agreement is cancelled, any outstanding balance for

treatment already carried out, or committed to, becomes immediately payable by the

Patient directly to Transform Dental.

Please note that any laboratory work, bespoke items, or customised products (including,

but not limited to, veneers, crowns, bridges, aligners, whitening trays, and retainers) arenon-refundable once ordered or commenced, as these are individually made to the

Patient’s specifications.

By proceeding with treatment, the Patient acknowledges and agrees to these terms.
